Description:		This document contains application for ethical approval form, informed consent form and participant information forms. We followed ethical issues according to the rules and regulation of the University since it is involved with human participants when conducting ELDP survey on Nyenkha spoken at Phobjikha, Wangduephodrang, Bhutan. We clearly made participants to understand how the language survey works, its purpose, their participation, benefits and risks and confidentiality of the study. We also tried to get the consents from not only from the main participants but also people who helped and involved in conducting this Nyenkha documentation project.
		Wangchuk is a PhD researcher at the University of Essex. He is PI, from Phonjikha and a native speaker of Nyenkha.
		Wangchuk is a PhD researcher at the University of Essex. He is a native Nyenkha speaker and the principal investigator of Nyenkha language documentation project for ELDP. All the actors (PI, RAs, helpers and consultants are involved in making this Nyenkha-Dzongkha-English mini elementary.
		Ugyen works for the DDC office and employed as RA with Wangchuk Rinzin. He is a native of Tshangla and uses Dzongkha and English as the national language and international language, respectively.
		Yangchen is a student and lives in Thimphu, the capital of Bhutan. She is a native speaker of Kurtokha and fluent in Dzongkha and employed as a helper for this Nyenkha documentation project - ELDP.
